During your first appointment, your provider will:
This comprehensive evaluation helps us understand your unique health concerns. Based on this discussion, your provider will order lab tests to gain further insights.
Your second (and possibly thrid) appointment will take place once we’ve received your lab results (2-3 weeks after your initial appointment) and focus on reviewing those results:
***Please note: If Specialty Testing (Carroll Food Intolerance or Mold/ Environmental Toxin related testing) is ordered in addition to standard labs, a separate Lab Review appointment will be scheduled to review those results, as they have extensive results and more detailed.***

Please note due to case complexity each follow-up can only accommodate one issue/concern:
 Acute Care: For established patients only- A focused, 15 minute visit for a minor illness or minor concern. If your issue requires more in-depth care, we may schedule a follow-up or refer you as needed. Not intended for assistance with complex illness issues, please book a Health Plan appointment instead.
Medication Management: For the management of medication prescribed by providers outside of PCNH. Not intended for Pain Medication Management
What to expect:
Initial Medication Management Visit – 60 min: Comprehensive evaluation of medications, history, and treatment goals. Lab orders may be placed if needed.
Lab Review Visit – 30 min: Review of recent labs and adjustments to medications if required.
Medication Refill Visit – 15 min: For stable patients needing medication continuation without major changes.
